How to Log In Step 1 – The first step to accessing your online account is to go to the main account management sign-in page (www.my.petinsurance.com). Step 2 – Enter your ID and password into the form on the left side of the page. Forgot Password – Click the ‘Forgot You Password?’ link on the sign-in page. You will be taken to the password reset page. Enter your ID, ZIP code, and name of your pet. Press the ‘Submit’ button to continue the process. Step 2 – In the first section, select your state from the drop-down list. Step 3 – Click the ‘Download Form’ button to download the claim form. Print the form out. Step 4 – After your trip to the veterinarian, fill out the claim form (pet’s diagnosis, total costs, etc.).
